Code of Conduct *- Code of Conduct forms should be printed out, read and signed by all parents, players, coaches and managers. SGS will adhere to the guidelines set forth in the code of conduct.
Criminal Background Check *- Criminal Background Check must be filled and returned to SGS with a copy of your drivers license. All manager, coaches and team moms are required to fill out a criminal background form.
Work Deposits *- Work deposit form should be printed out, read and signed by every family registering a child to play. This ensures you understand what is required to receive your refund. Refunds will be issued after the end of our season.
